Position Description

CHC Services is a well-established and highly respected Home Health and Home Care agency servicing the Central Puget Sound region.  Due to our large increase in our client base, we are looking for a RN to join our Clinical team with a client comes first attitude.

We provide a wide range of care with some of our current client care including spinal cord and brain injury, bone and joint injury, infusion therapy, and ventilator care. We also provide Care Management services. Preferred candidates will have a wide range of experience in many areas of nursing.

Duties, Responsibilities, & Accountabilities of the Full-Time RN Including but Not Limited To:

  • Assure that all of the client’s needs are being addressed in a timely and clinically expert manner
  • Performs the essential activities of Skilled Nursing visits for our Home Health Clients
  • Assessment, planning, implementation, coordination, monitoring, evaluation, outcomes, and general interventions of skilled nursing practice and process and makes appropriate referrals to other disciplines.
  • Responsible for Home Health opens
  • Responsible for RN delegations
  • If a caregiving shift is unable to be filled or if the caregiver in the shift doesn’t have the skills/credentials will work the shift or part of the shift in an emergency
  • Performs the initial visit to a client, completes all required paperwork, implements care and reviews and revises the plan of care as needed.
  • Assures the Plan of Care is followed.
  • Supervises and documents the employee’s performance every 90 days and completes supervisory visits• Develops and assists with the yearly employee evaluations
  • Provides on-call services after hours and weekends as needed or directed by the Administrator.
  • Promotes effective Home Care services in compliance with agency philosophies, policies and procedures, and in accordance with State licensing laws and regulations.
  • Works closely with office staff to promote inter-agency and intra-agency communication to ensure appropriate client care
  • Observes client for changes in condition and reports such changes to the Physician and/or Clinical Director & Clinical Supervisor, obtains orders for changes to the Care Plan and documents all observations and interventions in the clinical record.
  • Develops and keeps current orders for medications and treatments as prescribed by the Care Plan
  • Coordinates and participates in the development and revision of patient care policies as they relate to the services provided by the agency.
  • Uses appropriate preventive, therapeutic and rehabilitative nursing procedures and keeps client's physician, family and the Clinical Supervisor informed of care being provided.
  • Assists and instructs client and family members in nursing care required to promote client's health and safety.
  • Documents nursing observations, interventions and evaluations of the care being provided in accordance with agency policy.
  • Participates in team conferences, as requested, and attends staff in-services programs.
  • Thoroughly explains the Company’s mission and value to the client and client’s family members, doctors, providers, etc.
  • Explains the different roles and services of the Company, describes client’s rights, and provides contact information for complaints
  • Researches client information regarding diagnosis and treatment options as requested
  • Researches medical history, care and/or equipment costs, community and/or government programs and other information that would contribute to or enhance cost-effective quality care for the patient.
  • Responsible for accurate, timely, and ongoing evaluation and reporting of client’s needs, services, and outcomes
  • Obtain signed service agreements
  • Address emergent nursing issues
  • Resource for client and staff to answer and advise on health related issues or care of clients
  • Demonstrate the company’s culture of compassion, accountability and results by displaying the highest standards of clinical integrity, superb follow through and consistency, and exceptional customer service and client satisfaction.
  • Supervise Care Specialists (CNA, HCA, & NAR’s)
  • Performs other duties as required.

Qualifications – Knowledge Required of the Full-Time RN:

  • Must be a RN in the State of Washington and graduated from an accredited School of Nursing.
  • 2 to 3 years’ clinical experience; preferably in rehabilitation, home care, hospital, or hospice
  • Must demonstrate an ability to practice independently in the delivery of nursing care.
  • Ability to read and write in English.
  • Require reliable vehicle for use when traveling to worksites
  • A valid Washington State Driver’s License
  • Proof of current auto liability insurance
  • Computer literate with experience in different computer programs
  • Strong grasp of legal and ethical issues pertaining to confidentiality.
  • Creative, flexible, and innovative.
  • Commitment to excellence and high standards.
  • Excellent written and verbal communication and presentation skills.
  • Customer service oriented
  • Ability to apply problem-solving techniques to the skilled nursing process.
